Benzodiazepines in Anesthesia
Mechanism, clinical uses, adverse effects, and flumazenil reversal of perioperative benzodiazepines.
01Mechanism and clinical effects
Benzodiazepines are positive allosteric modulators at GABA-A receptors. They increase the frequency of chloride-channel opening in the presence of GABA, producing anxiolysis, sedation, anterograde amnesia, anticonvulsant activity, and muscle relaxation.
Midazolam is water soluble in its acidic formulation and becomes more lipid soluble at physiologic pH. It is used for premedication, procedural sedation, induction adjuncts, and seizure treatment, with dose and route individualized to the patient and setting.
02Adverse effects and interactions
Benzodiazepines cause dose-dependent sedation and ventilatory depression. Older adults, patients with limited respiratory reserve, and those receiving opioids or other CNS depressants are especially vulnerable.
- The combination with opioids can produce profound sedation, respiratory depression, coma, and death.
- Hypotension is more likely with rapid administration, hypovolemia, or other anesthetics.
- Hepatic dysfunction and interacting drugs can prolong midazolam exposure.
- Paradoxical agitation can occur but is uncommon.
03Flumazenil
Flumazenil competitively antagonizes the benzodiazepine binding site. It can reverse sedation and ventilatory depression attributable to benzodiazepines, but it does not reverse opioids or other sedative classes.
Its duration may be shorter than that of the benzodiazepine, so resedation is possible. Flumazenil can precipitate seizures or withdrawal in benzodiazepine-dependent patients and is hazardous in selected mixed overdoses, particularly when a proconvulsant coingestant is suspected.
